In Canada

By becoming a registered organ donor, you may save a life someday. There is a chronic shortage of most organs needed for transplant in Canada and the United States– currently, more than 4,500 Canadians are waiting for organ transplants, and too many of them are children.

Organ and tissue donation provide each of us with a special opportunity to help others. Sharing your decision to be an organ donor with your family is as important as making the decision itself. Sharing your decision now will help your family to carry out your decision later. It is necessary for you to document your decision by completing your registration in your resident province.

British Columbia

To become an organ donor in British Columbia, you can register online HERE.

Hard copy registration forms are available at all Service BC offices, ICBC offices, BC Transplant head office. You can also download a form HERE.

Alberta

The Alberta Organ and Tissue Donation Registry was created in April 2014. There are two ways you can register to become an organ and tissue donor:

  1. You can register online at UltimateGiftAlberta.ca.
  2. You can register at a registry agent office. Over 90% of registry agents in Alberta can help you register to be a donor.  All registry agents should have this ability by March 2015.  More information is available HERE. 

Ontario

To become an organ donor in Ontario, you can register online HERE, register in person at all ServiceOntario centres, and register by mail.

To register by mail, complete the Gift of Life consent form and send to:

Quebec

To become an organ donor in Quebec, have your decision recorded in the Registre des consentements au don d’organes et de tissus of the Regie de I’assurance maladie du Quebec (RAMQ). You can obtain the required form HERE.

You can also sign the sticker and put it on the back of your health insurance card. The sticker is sent to you at the same time as your health insurance card at renewal time. You can obtain the sticker from many health-care institutions in the system. You can order or download the sticker click HERE.

Nova Scotia

To become an organ donor in Nova Scotia, visit Legacy for Life’s website at http://www.legacyoflife.ns.ca/ to download the form or call MSI at 496-7008 in HRM or toll-free at 1-800-563-8880 to do it through your health card.

New Brunswick

To become an organ donor in New Brunswick, you must indicate your intent to donate on your Provincial Medicare Card. You can do this by calling Service New Brunswick at 1-877-762-8600 and select “2” to request your Medicare Card to be updated. Your new Medicare Card will clearly display a “D” to indicate you are a donor. You may change or withdraw your consent to donate at any time.

Discuss your decision with your family or loved ones. It is important those closest to you know your wishes.

To become an organ donor in Manitoba, you can register online at signupforlife.ca. Not sure if you are registered? Check your registration status HERE.

Prince Edward Island

To become an organ donor in Prince Edward Island, please visit Prince Edward Island Health HERE.

To register online, please click HERE.

You can also register your intention to become an organ and tissue donor, by completing the Intent to Donate Form and mail to Medicare Office, 126 Douses Road, PO Box 3000, Monatgue, PE  C0A 1R0.

Saskatchewan

To become an organ donor in Saskatchewan, you can now register online.

In Saskatchewan, organ and tissue will not be donated without your family or next-of-kin’s consent. Talk to your family about your decision to donate.

Newfoundland and Labrador

To become an organ donor in Newfoundland, visit Eastern Health’s website at http://www.easternhealth.ca/Give.aspx?d=1&id=323&p=53  or call 709-777-6600 or Toll-free 1-877-640-1110.

Yukon

To become an organ donor in the Yukon contact the Yukon Organ Donor Program at 867-667-5209; or 1-800-661-0408, local 5209.

Northwest Territories

The back of your Health Care card is the donor card. To receive your Donor’s card contact Alberta’s:
